* Name, date and place of birth, address, ID Card/Passport.


* Leave empty, if you want to register single-person limited liability company.
* Specify whether the capital as of the moment of incorporation will be paid up in whole or in parts (min. 35 %).
* e.g. 50 shares with a value of 100 levs each.

* If there is such, please provide its detailed description.

* Please, attach all available related documents, such as current Articles of Association, document for registration of a shareholder - foreign business entity.
